Garcinia cambogia extract (GCE) comes from a tropical fruit from India and Southeast Asia. The active ingredient, hydroxycitric acid (HCA), is said to block fat and suppress the appetite.
Dr. Oz has promoted a series of weight loss supplements on his show. All that sounds too good to be true, and it is. Garcinia probably does work to some extent to improve weight loss, but the evidence doesn’t begin to justify such grandiose claims.
Garcinia cambogia extract (GCE) comes from a tropical fruit grown in India and Southeast Asia. The active ingredient has been identified: hydroxycitric acid (HCA). It is said to block fat and suppress the appetite. It inhibits a key enzyme, citrate lyase, that the body needs to make fat from carbohydrates. It suppresses appetite by increasing serotonin levels; low serotonin levels are associated with depression and emotional or reactive eating.
It allegedly decreases belly fat, suppresses appetite, controls emotional eating, and changes body composition by increasing lean muscle mass. It doesn’t just produce weight loss, but it improves overall health. It is said to decrease cholesterol, LDL, and triglycerides by 10-30% and to raise levels of the “good cholesterol” HDL.

Oz recommends a dose of 500-1000 mg of Garcinia cambogia potassium before each meal. (Some experts believe the potassium salt is more effective than other formulations.) He says not to take it if you are pregnant or breastfeeding, or if you have Alzheimer’s disease or other forms of dementia, because it can worsen dementia. If you have diabetes, it can cause hypoglycemia, and if you are on a statin, it can increase the risk of harmful side effects like rhabdomyolysis or muscle degradation.
Oz contradicts himself: he suggests that it can produce weight loss without diet or exercise, yet he clearly recommends it be used along with exercise and properly portioned meals.
The garcinia evidence
With Dr. Oz’s track record, I was not willing to simply accept his word for the wonders of Garcinia. I went to PubMed, where a search for hydroxycitric acid brought up 64 articles. Some were irrelevant, and the relevant ones included a lot of animal studies and a smaller number of human studies with inconsistent results.
Guinea pigs on a high cholesterol diet who were given a different Garcinia species (atriviridis) had a tendency to decrease lipid composition levels and fat deposition in the aorta. HCA caused congenital defects in rats. Another rat study found that it decreased body weight gain and visceral fat accumulation by reducing food intake but had no lasting beneficial effects on hypertriglyceridemia and hyperinsulinemia. Yet another rat study showed that it suppressed body fat accumulation but was toxic to the testes.
To try to make sense of the inconsistent results, Onakpoya et al. did a systematic review of the published randomized controlled trials (RCTs) as of 2011. Their analysis found a small, statistically significant difference in weight loss (1.75 kg vs 0.88 kg, less than 2 pounds). They commented that the studies all had methodological weaknesses, so these results could be due to GIGO (garbage in/garbage out). The two studies with the best methodology found no statistically significant difference from placebo. Adverse events were twice as common with Garcinia (headache, nausea, upper respiratory and gastrointestinal symptoms). The authors concluded:
The evidence from RCTs suggests that Garcinia extracts/HCA generate weight loss on the short term. However, the magnitude of this effect is small, is no longer statistically significant when only rigorous RCTs are considered, and its clinical relevance seems questionable.
Oz featured a patient testimonial on the show from a woman who had lost 10 pounds in 4 months. She started noticing results after a week; she reported that her sugar cravings were decreased, she had more energy, and she went down a dress size from 10 to 8.
Being fat may also be a medical issue such as hormones running wild, which you have little control over and needs medical intervention.

At this point, I don’t think we can reliably say whether Garcinia has a clinically relevant advantage over simple calorie reduction and exercise. It appears to be safe, and it may have a role in helping patients lose weight by assisting motivation and enlisting placebo effects.
Dr. Oz’s popularity is enduring, but diet fads aren’t. I confidently expect another “miracle” to supplant Garcinia in the Land of Oz in the not-too-distant future.

Garcinia Cambogia Weight-Loss Pill Is No Miracle
The claims make this supplement tempting, but they're untrue
Garcinia cambogia is hot. Nearly a million Americans each month Google this supposed weight-loss supplement. They're looking for reviews on garcinia cambogia's effectiveness, what kind of side effects it causes, and where they can buy it. My mom recently bought a bottle of the pills at Costco because she saw a segment about garcinia cambogia on a TV show.
Manufacturers claim that garcinia cambogia boosts weight loss by, among other things, "slowing the body's ability to absorb fat," "replacing fat with toned muscles," and even improving your mood and suppressing "the drive to react to stressful situations with food." How, you may ask? It's mostly pinned on hydroxycitric acid (HCA), a substance found in garcinia cambogia that appears to inhibit an enzyme called citrate lyase and interferes with fatty acid metabolism.
“HCA does do that—but in a petri dish," says Steven Heymsfield, M.D., the former head of the Pennington Biomedical Research Center in Baton Rouge, La. "Converting that to actual weight loss in humans would take 1,000 steps beyond that," he says.
Back in 1998, Heymsfield published the first randomized controlled trial on the effectiveness of garcinia cambogia, in the Journal of the American Medical Association.
Since the tropical fruit can be a bit difficult to obtain in the US and other western countries, the fruit extract is the next best thing that you can take advantage of.
He found no weight-loss benefits. Heymsfield, who continues to study the topic of weight-loss supplements at Pennington, says that about a dozen negative studies have since been published about garcinia cambogia. But that has not stopped marketers of the supplement, he says, from “weaving a story with obscure facts. Maybe each fragment has some validity, but if you wind it together it makes no sense at all.”
His original study, conducted by Columbia University’s Obesity Research Center, looked at 135 overweight men and women age 18 to 65; about half were given garcinia cambogia and the other half a placebo three times a day before meals. Both groups ate a high-fiber diet and returned for evaluation every two weeks. At the end of the 12-week trial, there were no important differences in weight loss between the two groups.
A review of 12 trials involving garcinia products published in the Journal of Obesity in 2011 came to the same conclusion. Another study by researchers at Victoria University in Melbourne, Australia, and published in 2013 in the journal Complementary Therapies in Medicine found that overall the evidence for garcinia cambogia was “not compelling.”

As for garcinia cambogia's side effects, controlled studies and animal studies have found very few, although Heymsfield says, “I don’t think it’s 100 percent safe.”
In 2009 the Food and Drug Administration warned consumers about Hydroxycut, a product line containing garcinia cambogia and several other ingredients, based on serious reports of health problems, including jaundice, elevated liver enzymes, liver damage requiring a transplant, and one death from liver failure. The FDA said it was unable to determine exactly which ingredients were associated with the liver injuries. (Hydroxycut's manufacturer, Iovate Health Sciences, withdrew the products, though it has since returned a reformulated product to the market containing no garcinia cambogia.)
“Being obese is difficult because only some of it is related to self-control,” Heymsfield says. “And it’s not easy to lose weight in our environment. Just preventing further weight gain is an accomplishment for some people.” The biggest problem with garcinia cambogia, Heymsfield says, besides being a waste of money, is that it distracts people from concentrating on the important things when it comes to weight loss: increasing your activity level and eating a healthier diet.
